Review personal/family health history, medications, injuries, pain, preventive-care status, and current limitations.
Track 7 days of sleep, movement, nutrition, hydration, energy, recovery, symptoms, and substance exposure.
Record simple baseline measures when appropriate: blood pressure, resting heart rate, weight/waist trend, and current activity level.

Age- and risk-appropriate preventive exam and clinician-guided lab work when indicated.
STOP-BANG or another appropriate sleep-risk screen when sleep apnea risk is relevant.
Safe functional baseline for aerobic capacity, strength, mobility, balance, and movement tolerance.
